CJBL Terms and Conditions

 I/We agree to return uniforms and other CJBL  furnished  property at the end of the season or upon request.  I/We further acknowledge that with the exception of High School Baseball or American Legion Baseball this applicant will not play in any other baseball league while participating at CJBL.  I/We recognize that if they do participate in any other program, they will not be able to continue at CJBL.  I/We further acknowledge the inherent risk in parking at CJBL facilities and that parking for my/our vehicle(s) at those facilities is at my/our own risk.         I/We, the parents of the above named participant, who is a candidate for a position on a team within a league operating under the name of CHATSWORTH JUNIOR BASEBALL LEAGUE INC. (AKA CJBL), hereby give my approval to his/her participation in any and all of the activities of said baseball league during the coming season.  I/We assume all risks and hazards incidental to participation in these activities and transportation to and from the activities.  I/We do further release, absolve, indemnify and hold harmless CJBL, and said league organizers, sponsors or any of the supervisors, any and all of them.  In case of injury to my son/daughter, I/We likewise waive, to the extent not covered by liability insurance, any claim against any person transporting my/our son or daughter to and from the activities.    In the event of injury or illness to the above named applicant, while participating in the above named baseball program, I/We hereby grant authority to a qualified physician to render such medical treatment to the applicant as said physician deems necessary under the circumstances.  Treatment is to be administered at the nearest or most appropriate medical facility.         I/We, the parents of this above named applicant, do further recognize and acknowledge the Chatsworth Junior Baseball League Inc. DOES NOT HAVE any primary Accident or Medical Plan Insurance.
